This book explores inter-professional rehabilitation team collaborations of healthcare providers & stroke survivors from onset of stroke through to discharge. It will look at inter-professional team decision-making and how the determination of a discharge destination for a stroke survivor is made. Inter-professional teams must deliberate weekly utilizing the voice of all team members including the stroke survivor in order to come to a consensus that is in the best interest of the stroke survivor. As a nursing professor, it has been my privilege to meet numerous patients and colleagues. It is through these relationships this book has emerged. Thus the impetus for this book and my area of research has been on inter-professional team collaboration and chronic disease management and how clinicians involve patients in their care trajectory.
